Parish Survey
This weekend, the Basilica is conducting a parish life survey to get to know our community better.
This is the fruit of a months-long initiative and it is a great opportunity to share what you love about the Basilica, what concerns you may have, and what you hope for our community moving forward.
This survey is for everyone age 16 and over who is either registered at the Basilica, or who regularly attends Mass or community events at the Basilica.
Everyone is encouraged to fill one out – even if you live in the same household as someone else completing a survey.
The survey will take 10 to 15 minutes to complete, all results will be anonymous, and there are both paper and online options.
Paper copies of the survey and submission boxes are available at the back of the church and in the lower level during coffee and donuts.
Checkboxes can be marked with an “X” or a check – you do not need to fill in the boxes completely.
If you would prefer to complete the survey online, links and QR codes to it are in the bulletin and posted on signs in the lower level.
We ask everyone to complete the survey this weekend.
If you are unable to complete yours today, or if you know someone who should be completing one who is not here, we will be accepting late submissions through Memorial Day.
If you fill out a survey by hand, you can drop it off at the Parish office or place it in one of the submission boxes until then.
We are looking for as widespread participation as possible – so please encourage everyone you know at the Basilica to fill one out (and do not forget to submit one yourself)!
